probably UMLS is the best, check here:

another option is using ICD-11 for diseases and ICHI for medications.

As far as I have understood, ICD-11 is available in many languages, but needs a separate installation, see docker instance instructions

ICHI is still in beta and I can't find any API except this explorer maybe we could ask the University of Udine if they have an API for that

preliminary tests have shown that the UMLS API is terribly inefficient! The reason is because for each term we would need to issue several requests which would take several seconds to be executed. Maybe a local installation would be faster but it requires at least 40GB of disk space! see this

tests preformed on the public API of ICD show better results
