Since we need to support the range of real-world use cases, I agree, supporting all variations of duplication makes sense. We should enumerate the possibilities for file duplication within a version and across versions.

I think this can be closed now: 1-7 are supported in the current draft. 8 has been ruled out-of-scope in OCFL version 1 (per OCFL/Use-Cases#27 and defer of #22)
