The potions in Reliquary are brewed differently to vanilla and the ingredients also work in a different way. Player first need to combine potion ingredients in mortar and use it to get potion essence, then the potion essence is used in cauldron to get the potion, that is taken out of the cauldron with empty potion vials.

Apothecary Mortar

Apothecary Mortar is the first step in potion brewing. You can combine 2 or 3 ingredients in the mortar and then use it by holding or pressing right click. The result of that is a potion essence that can be used further in cauldron. The ingredients have to be different from each other and one of them can be potion essence that was created previously.
Each of the reliquary potion ingredients has multiple passive effects assigned to it, but these will only make it into the potion essence if you combine ingredient that share the same passive effect. Their potency and duration is combined from all of the ingredients and there is a bit of a loss of duration in the process. So you cannot endlessly combine potion essence with potion ingredients to produce insanely long running potions.
Potion ingredients and their passive effects are configurable and below is a list of those in default configuration.

Apothecary Cauldron

Apothecary Cauldron is the block used to brew the potion. It needs fire underneath it and water inside to be ready for potion brewing. Then you can put in potion essence and finalize the potion with nether wart. It will then start showing some sparkly particles and potions can then be taken out with empty potion vials. There are 3 potions brewed per brewing similar to vanilla.
In addition to just plainly brewing drinkable potion one can also brew splash potions by adding gunpowder just before nether wart.
And potions can also be made more potent and durable by adding glowstone (up to 4) and redstone (up to 5). Just keep in mind that these do less with every additional one you put in. And in case of glowstone you lose quite a bit of duration to the point where some of the potions may not make sense because their effect will run out very quickly.

Ingredients and their passive effects

The tiers just divide these in groups of similar power ingredients. With higher tier you're getting longer durations and higher potency.
"+1" next to duration stands for potency augment - basically when combined the effect starts as level one potency and gets +1 to potency for each ingredient that has the effect with +1.
