Hmm, from the photos I don't see anything wrong. Did you try hitting "refresh cache of current page"?
Also what version of the program are you using?

Yes, I already did. I'm using OBS 27.1.3 and also, I made it work first time and after trying to do it again it keeps failing on me.

Mine is doing the exact same thing, I can get it to work on my gaming PC but not my stream PC. I think it may have something to do with a codec?
On the gaming PC it plays fine in chrome. on the stream PC it plays a white box and nothing else when I open the html file directly

Also, I've tried setting hardware acceleration differently in obs and in chrome.
