Document the fact that "pipeTableEither" skips a row #133

pipeTableEither skips one row of input, presumably because it's expected to be used in a context where the first row is a header. But that behavior is undocumented and not really what you expect from the signature.

I'm guessing a bunch of the other pipe functions do this as well?

It should be documented or have a boolean argument or something, I think.

It's possible that this is addressed in the fixes for #116
?

But I can't quite tell.
