Add functionality to clear Physical Address fields and Alternate Address fields on MyProfiles

On the MyProfiles page of the PWA where participants are able to edit their User Profile data, please add some type of 'clear' button or functionality on the 'Edit' screen for the fields for 'Physical Address' and 'Alternative Address'. Where when the participants selects 'clear' the original data is moved from the current non-nested variable structure to the User Profile History nesting structure and the current address fields reflect no data entered.

• This functionality should only be added for those with verification status = Verified.

• Note: 'Alternative Address' are new fields being added to the UP in the February release.

This functionality is only needed for Physical Address and Alternate Address. Other fields (e.g. name, preferred email, mailing address, etc) are either required and only move to the UP Hx nesting once new data is entered or the field is not required (e.g. alternate contact, additional email 1 and 2, etc) so participants can just delete the data and save it. No new functionality is needed for these.
