The install path for macOS is now under:

${HOME}/Library/Application Support/org.inkscape.Inkscape/config/inkscape/extensions

This is the only place I have found on a modern installation (macOS 14/Inkscape 1.3) where a user has the correct permissions to write and Inkscape will pick up the extension.
