As of Nov 3 2017, the AppVis extension shows up without an icon.

Odd behavior, but I managed to fix it.
I simply changed the path for the extension image to use my repo instead of Farzeen's.
Icon appears as it did before now, extension will need to be updated on AI2 projects for the image to appear again.
