transaction vs rollback/commit

Perhaps I have overlooked something - forgive me if I have.
I have tried the transaction block. Even if I throw an exception within it, the transaction is not rolled back. I also cannot see a way to trigger a rollback programmatically.
Having this in place would greatly improve the integrity of our data.
