Bug Fixes:

• Fixed bug that allowed rows without a survey id to be inserted into database.
• Fixed bug where module would not account for dashes in the event name, therefore throwing an error that suggested the event didn't exist.
• Fixed bug where checkboxes were being flagged as invalid in the branching logic.
