This pull request introduces changes and additions to the utility mpas2esmf in
MPAS-Tools/mesh_tools/mpas2esmf. mpas2esmf produces both an ESMF description of the MPAS atmosphere mesh and a SCRIP description of the mesh. These descriptions are needed by ESMF within the Community Earth System Model (CESM) for interpolations to and from the MPAS Voronoi mesh.
This pull request corrects the definition of the mesh refinement factor needed needed by the CESM/CAM TOPO tool so that the terrain is appropriately averaged over MPAS mesh cells on variable-resolution meshes.

This utility produces both a "scrip" (depricated) file and an esmf
mesh file compatible with CESM2.

Corrected the computation of the refinement factor and also have the
computation use the mesh density function.  The refinement factor is
used by the CESM/CAM topo tool to determine how to average the
topography and compute the subgrid topography variables for the CAM6
gravity-wave drag scheme.
