Documentation cleanup (#23)

* Add CHANGELOG.md to the generated YARD docs
* Add documentation link to README.md
* Change the GitHub Actions workflow name from "Ruby" to "CI Build"
* In the Gemspec, reference CHANGELOG.md on RubyGems.org
